Become a TBI Waiver Provider

• Contact RRDS in Buffalo region @ (716) 629-3633

• Send on letterhead, request to provide specific service and include history of service and of agency

• Complete application packet and submit to RRDS

• Attend scheduled interview with RRDS

• Review of RRDS recommendations by DOH

• Receive final determination from DOH
